Woodside

HFE review of the design and installation to replace and relocate safety critical plant. The primary focus was to optimise maintenance access, equipment monitoring and reliability to reduce the risk of human error and personal injury. Findings were provided to either confirm compliance, or provide recommendations for improvement, against the Woodside HFE Design Standard. HFE review to support the installation and upgrade of new plant and equipment. This included attendance to HAZID workshop, design review meetings and delivering a human factors workshop.

Providing Human Factors support for the transition to Autonomous Haulage System (AHS). This included Alarm Management, Human Machine Interface (HMI) design and layout within the cab and supporting the client to build a more robust set of design criteria around which the contractor can then design the system for optimal usability.

This will focus on the design of the Integrated Remote Operations Centre and will include: Development of the human factors integration plan, workplace ergonomics review, useability trials, review of procedures and user manuals, site based validation studies and final human factors evaluation.

Coretex Consulting have been providing support to the AMPOL Future Fuels Project which aims to reduce sulphur from 150ppmw to 10ppmw across all fuel grades at Lytton Refinery. Principal Human Factors Consultants at Coretex Consulting have been provided support across 3D model review and dedicated Human Factors Workshops. These Human Factors Workshops aim to take risk based approach to review and optimise operability and maintainability of the work environment using design drawings and 3D Models. It also aims to minimise the likelihood of human failure and mitigate against its consequences.

Analyses of safety critical activities to identify major accident hazards and the associated critical tasks. We then assessed process steps which relied on the operator to perform them correctly and the types of errors which may occur. This resulted in a more thorough understanding of these steps and more effectively controls developed to ensure error tolerance and improved process reliability. Advice was also provided on improvements for the procedure and support work instructions.

Review of Nickel West’s HSE Management System and supported development of their Human Factors Integration Plan to prepare for submission of a Safety Case to DMIRS. Key focus areas included: Safety in Design for current and future projects, incident investigation process, prevention of MAE’s, job planning, the permit process, Field Leadership, control room design (HMI) and operation, and the management of multiple small brownfields projects within the existing plant.

Conducted a human reliability study for safety critical tasks offshore to identify opportunities for improved design controls. As well as the task analysis human reliability was also assessed within the project risk register to improve project risk control measures, bowtie analysis and learnings for future projects.

Diamond Drilling

Conducted a human factors review via a site visit and HAZID workshops of drilling operations, specifically an analysis of the risks and controls associated with non-automated fingerboard and rig floor operations. Also included was the assessment of derrick maintenance and evacuation. Key areas of focus included: fall from heights, dropped objects, handling tubulars and the consideration of automating drilling operations retrospectively and the effectiveness of this when considering the interface with existing plant and equipment.

Conducted a human factors assessment and supporting issues register to improve understanding and execution of human reliability studies, change management and frontline risk management. We also conducted a broader human factors assessment to support compliance with regulatory requirements with a focus on psychological risk management.

Providing human factors and ergonomics support to the Chevron Global Architect division and Australian Business Unit to optimise the design of their new headquarters in Perth and achieve Platinum GreenStar rating. This included workstation design and selection, and the broader workspace design to optimise collaboration and reduce the musculoskeletal risk associated with prolonged sedentary postures.
